Patient registration
The registration form collects your contact details, preferred clinic, health card, health overview and consent so authorized Valleyview staff can review your request and contact you. Health card numbers are encrypted before storage. Staff reports require sign-in and are limited to authorized clinics. A registration is not a confirmed appointment or acceptance into a practice.
Clinic assistant
Chat messages are processed by an AI service to answer public clinic questions. Do not include names, health card numbers or personal medical information. The assistant cannot access registrations, provide medical advice or change appointments.
Your browser and other services
Your selected clinic is saved in your browser. Staff sign-in uses a secure session cookie. Hosting and security services may process technical request information. Maps load through Google, and booking opens Ocean; those services have their own privacy terms.
