Our New Registration Process

To simplify the registration process, we kindly request that you complete the form in person, as we are unable to send it via email. Due to high demand, we have temporarily suspended online patient registrations. We are still accepting new patients — please register in person at any of our clinic locations. What to bring: • Valid Ontario health card • Government-issued photo ID • List of current medications • Any relevant medical records or referral letters Our friendly staff will assist you with the registration process. Walk-ins are always welcome during clinic hours.
