Your health card is required for every visit to Valley View Medical Centre. Please ensure you carry a valid, up-to-date health card with you at all times.
Required for Every Visit
We kindly request that you bring your health card to each appointment. Our receptionists will ask you to present your health card during the check-in process for every visit. It is your responsibility to carry a valid and up-to-date health card.
Ontario Health Card Policy
At Valley View Medical Centre, your healthcare services are covered through your health card. According to the policy of the Ontario Ministry of Health, a valid provincial health card (Ontario or any other province) is mandatory for each visit.
Failure to Present a Valid Health Card
Failure to provide a valid health card at the time of your visit may result in charges based on our list of Uninsured Services. For further information, please refer to Valley View Medical Centre's Uninsured Services list as of January 1, 2023. If you are able to present a valid health card within 30 days of your visit, any amount you have paid will be reimbursed, excluding our administrative fee.
Keeping Your Health Card Up to Date
Keeping your health card up to date is crucial to ensure uninterrupted access to healthcare services and to prevent any potential fraud.
In case of a lost, stolen, or damaged health card, please promptly contact the Ministry of Health and Long-Term Care information line at 1-800-664-8998. If you have experienced changes in your address or name, please visit or contact your local OHIP office or the Ministry of Health.
